Focal Fatty Infiltration

  1. Pathogenesis:
  • Fat focally accumulates in hepatocytes, creating low-density regions seen on CT, which mimic a low attenuated metastasis.
     
  • More commonly found in the left lobe of the liver and adjacent to the falciform ligament.
     
  • Associated with alcohol abuse (most common), obesity, steroids, and hyperalimentation.
  1. Radiographic findings:
  • U/S: bandlike geographic areas of increased echogenicity in a lobar/segmental distribution.
     
  • CT: no mass effect on adjacent vessels, patchy areas of decreased attenuation.
