Case Scenario and Question – Appendicitis

Question:   A woman at 14 weeks gestation has developed right lower quadrant tenderness in the last 12 hours. Abdominal ultrasound and MRI are inconclusive. In this context, which is FALSE:

    A.  Multi-detector CT (MDCT) for appendicitis may expose the fetus to 20 to 40 mGy, approximately 6 times higher than a renal stone protocol with MDCT.
    B.  The safety of both iodinated IV contrast and gadolinium in pregnancy is unproven. 
    C.  MDCT for appendicitis is below the threshold for increased cancer risk

    D.  Ultrasound of appendix is difficult in first trimester and nearly impossible by the third trimester.
